In Q2 of 2025/26 alone, over 809,000 patient safety incidents were recorded. That’s a staggering 96.96% of total events! Among these, a whopping 483,743 were classified as “no harm.” Great, right? Except that doesn’t change the fact that every one of those incidents could have been a disaster waiting to happen.

Nurses are crucial to healthcare, but staffing shortages threaten patient safety, with nearly 810,000 incidents reported in just one quarter.

Now, let’s talk about staffing. UNISON’s report reveals that 70% of shifts lacked enough nurses for safe care. That’s an uptick from 63% in 2023. So, it seems the situation is only getting worse. 69% of shifts had insufficient staff to ensure safe care, which only adds to the alarming trend. The rising workforce shortages in healthcare are contributing significantly to these staffing issues.

Safety concerns in maternity and elderly wards? They’re at 81-82%. Red flags are waving everywhere, with 56% of shifts reporting serious safety risks. Yet, only 21% of workers felt their employers would respond satisfactorily. So much for confidence.

And what about the nurses themselves? They face burnout and fatigue, which are notoriously bad for patient safety. With averages showing up to 2,660 flu patients in beds daily, it’s a nightmare. The numbers scream for attention, yet they often fall on deaf ears. Interestingly, the LFPSE service captured a total of 833,136 events, highlighting the critical need for improvement in patient safety.

Sure, there’s a 5.6% nurse turnover rate, which sounds manageable until you realize that incidents like falls and medication errors are on the rise.
