Alzheimer’s and other dementias contribute 10.4% to the neurological burden. Here’s where things get murky—risk factors only explain about 22.3% of Alzheimer’s DALYs. Scientists are basically shrugging their shoulders at the rest.

Some conditions show clear gender preferences. Migraine, multiple sclerosis, and tension headaches favor women, while ADHD, traumatic brain injury, and autism spectrum disorder hit men harder. Overall, males rack up higher age-standardized DALYs for neurological conditions than females.

Geography matters too. Stroke dominates most regions except Western Europe and Australasia, where apparently they’ve figured something out the rest of us haven’t. The disparities are particularly stark when comparing regions, with central sub-Saharan Africa experiencing over 7,000 DALYs per 100,000 people compared to under 3,000 in high-income areas.

The prevention story offers mixed results. Conditions like tetanus, rabies, and meningitis have seen dramatic declines thanks to better healthcare and prevention efforts. But diabetic neuropathy, cerebral malaria, and sepsis complications are growing faster than weeds due to poor prevention strategies.

COVID-19 has crashed the neurological party with long-term cognitive symptoms adding to the global burden. Because apparently we needed another neurological headache.

The bottom line? While some neurological disorders remain medical mysteries, many could be prevented with better risk factor management. In the United States alone, nearly 100 million Americans were affected by neurological disorders as of 2011, highlighting the enormous scale of this public health challenge. Unfortunately, knowing what causes these conditions and actually preventing them are two very different things.
